San Jacinto Hospice of the Heart, Inc. is a freestanding, non-profit, organization designed to care for the terminally ill in our community. Mission Statement To improve the quality of life of all people in each of the communities we serve in the environment of choice selected by the patient. Vision Statement Hospice of the Heart, Inc. will be the preferred hospice network integrating hospice services in each community that we serve and exemplify in all things the spirit of Christian compassion and love. In partnership with our patients, physicians, other care providers, community resources and affiliated organizations, we will consistently meet the needs and exceed the expectations of those we serve. |
